Acceptance of Terms
By scheduling an appointment or receiving services at Lakeside Dental Hartford, you agree to these terms of service. Please read them carefully.
Appointments
We value your time and ours. Please note:
- Arrive 10 minutes early for your first appointment to complete paperwork
- If you need to cancel or reschedule, please give us 24 hours notice
- Missed appointments or late cancellations may incur a fee
- Repeated missed appointments may result in dismissal from the practice
Payment and Insurance
Payment is due at the time of service. We accept:
- Cash, checks, and major credit cards
- Most dental insurance plans
- Payment plans for major procedures (subject to approval)
We will file insurance claims on your behalf. However, you are ultimately responsible for payment. Insurance is a contract between you and your insurance company. Any unpaid balance after insurance processes is your responsibility.
Treatment
We will explain all recommended treatments and obtain your informed consent before proceeding. You have the right to accept or decline any treatment. We recommend following through with prescribed treatment plans for optimal oral health.
Emergencies
For dental emergencies during business hours, call our office at (860) 555-0123. We reserve time slots each day for urgent cases. For after-hours emergencies, follow the instructions on our voicemail or visit the nearest emergency room.
Patient Responsibilities
As a patient, you agree to:
- Provide accurate medical and dental history
- Update us on any changes to your health or medications
- Follow post-treatment instructions
- Maintain good oral hygiene between visits
- Treat our staff with respect
Changes to Terms
We may update these terms periodically. Changes will be posted on this page with a new effective date. Continued use of our services after changes constitutes acceptance of the updated terms.
